SSBG Case Manager
Full-time (37.5 hrs/per week) with benefits

This position offers State of Tennessee Retirement (TCRS), State of TN Health

Entry level rate of pay – $14.50 – $16.50 per hour (based on experience)
Departmental Context:
The Senior Services Program provides services through the following programs: Nutrition, Foster Grandparent, Homemaker, Choices, SCSEP,
and SSBG.
Purpose
The SSBG Case Manager is responsible for serving consumers through the Social Services Block Grant (SSBG). This position is
accountable to the Homemaker Coordinator.
Position Responsibilities:
 Conduct assessments and/or reassessments for Homemaker Program clients to include
Dept. of Human Services Adult Protective Services, Title III, Caregiver, Options, CHOICES clients
 Develop care plan for DHS clients and conduct reassessments every six months for each SSBG client to
determine eligibility, reestablish the need of the protective service case, the level of care required, and/or
changes in status
 Make and submit recommendations to Homemaker Coordinator regarding delivery of services, and/or client
issues, etc.
 Maintain documentation of assessments, services provided and clients’ progress
 Coordinate services with other agencies and/or groups in an effort to fully utilize all resources available to
assist clients
 Accompany SSBG and SWHRA staff on home visits whenever there are issues or concerns that need to be
addressed as approved by the Homemaker Coordinator
 Participate in all in-service training for Homemaker Program staff
 Travel as required to perform assessment and monitoring activities
 Maintain accurate and timely documentation of all work related activities in an organized and confidential
manner to ensure compliance with DHS and Licensure guidelines
 Establish and maintain effective working relationships with clients, family members, service providers, co-
workers, the public, and agencies, including but not limited to DHS, AAAD, and MCO’s
 Responsible for assisting with yearly training of the SSBG Homemakers
 Perform other duties as required by the Homemaker Coordinator
Technical skills and Experience required
Qualifications, technical skills, experience, and knowledge required for this position include:
 Graduation from an accredited four-year college of university with a B.A. Degree, including or supplemented
by twenty-one quarter hours in behavioral sciences, i.e. social work, psychology, sociology, and/or home
economics (graduation from an accredited school of nursing and licensed to practice in Tennessee as a
registered nurse may be considered an acceptable substitute)
 Considerable experience working with elderly and disabled adults
 Knowledge of local and regional service providers for elderly and disabled adults
 Strong organizational, interpersonal and decision making skills

 Knowledge of federal and state regulations governing the delivery of services to the elderly and disabled
 Excellent written and oral communication skills, organizational skills, and handling people in a professional
manner
 Skilled in the use of Microsoft Office software (Excel and Word)
 Applicant must possess a valid driver’s license and the ability to be insured at

Transportation Van Driver

Rate of Pay $12.50 – $12.75 per hour

Full-time and Part-time positions available.

The Transportation Van Driver is responsible for transporting individuals in the service area to and from community resources.  This position is accountable to the Operations Supervisor. 

Position Responsibilities:

  • Responsible for the prompt and dependable pick-up and delivery of individuals to requested destinations
  • Responsible for the safety of passengers
  • Responsible for obeying all traffic laws and regulations to include no texting or cell phone use while driving
  • Responsible for the care, maintenance and the cleaning of vehicles both inside and out
  • In coordination with the office personnel, be responsible for the prompt completion of all required reports.
  • Maintain a safe and orderly vehicle at all times
  • Responsible for notifying area dispatcher, administrative assistant, operations supervisor or Transportation Director of any violations received affecting motor records, the same day it is received
  • Responsible for being friendly and courteous with co-employees and passengers at all times, also for assisting other staff in keeping office and other work areas clean and presentable
  • Responsible for any other duties as added by the Operations Supervisor and Transportation Director
  • Responsible for gaining and maintaining skills to operate the various vehicle technologies
  • Responsible for gaining and maintaining skills to operate the various lift equipment
  • Responsible for knowing agency Policies and Procedures, as well as to master and use correct techniques and to keep the operations supervisor/dispatcher informed of any problems that arise
  • Responsible for driving in any of the county service areas
  • Responsible for a pleasant personality and appearance, knowledge of defensive driving techniques and the ability to establish and maintain an effective working relationship with other agencies and the general public
  • Responsible for maintaining knowledge of skills and techniques updated by attending regular scheduled training sessions
  • Responsible for evaluating each situation, knowing when not to attempt to help when you are physically unable to do so and knowing when to request help
  • Assist both ambulatory older persons and wheelchair persons into and out of the vehicle.  (Drivers are not expected to move wheelchair persons up and down more than 1 stair and are not permitted to enter a passenger’s home)

Qualifications, technical skills, experience, and knowledge required for this position include:

  • Must be age 21 or older
  • Must have a minimum Class D Driver’s license with an F endorsement
  • Must be able to occasionally lift up to 50 pounds
  • Must pass a Motor Vehicle Registration (MVR) check, a criminal background check and an FTA (Federal Transit Administration) approved pre-employment drug test
  • Must be clear of all National Offender Registries, such as the Sexual Offender Registry and the Elderly or Disabled Abuse Registry
  • Drivers must be readily accessible by telephone, and be able to report to work on short notice and at odd hours
  • Must be a high school graduate or have a G.E.D.

Meals on Wheels Drivers

The Covington driver position consists of deliveries into the Covington, Brighton and Mason areas twice a week on Wednesday and Friday.
Halls delivery day is on Wednesday and covers the Halls and Gates areas.
